Use this form to request access (via key or WiscCard) to a room within a DoIT building, or if a room in a DoIT building needs to be re-keyed for any reason.

Please name the room(s) to which you need keys or access updates.

Please list names of the recipients of the keys or access. If WiscCard access is required, please list the first six digits of the sequence listed under the magnetic strip on the backside of the their WiscCard.
