Project Description

Official Project name. Briefly describe what you are planning to improve.

Who is supporting this project? The sponsor generally should be someone in a direct reporting relationship above at least one of the team members who has a strong vested interest in seeing that the project goals are achieved in the given timeframe.

Operational Theme*

Please select the operational Theme to which this project best fits.

Your understanding of what is problematic about the current state. What/where/when is the problem? What is happening that shouldn’t be? What needs to happen more frequently? What needs to change? Be specific, using data, if possible.

What is the context of the project relative to the overall vision and needs of the institution? How is the problem negatively impacting patients or the organization? What is the financial or other business case for the department? What safety implications are there, if any? Why is this project important?

What do you want to accomplish? Over what time period? Example: Decrease the amount of time patients wait from arrival to scan from 30 minutes to 15 minutes by Sept. 2020.

What data will be collected? Is the data currently being collected? If the data is not currently being collected, what is your plan to obtain it?
