Strong Minds PNF - Products Need Form

This Products Need Form should be completed by the SO Program Director in conjunction with the Strong Minds Clinical Director at least 60 DAYS PRIOR to the event date.

We have updated our policy for Strong Minds materials requests.

Programs who plan to screen >200 athletes should now order materials directly, either from our vendor or elsewhere. Our vendor's website is https://www.so-health.org/cool-tools.htm.

Funding for these materials can be requested through the Healthy Athletes Grant.
