This form can be used to apply for an Onsite Sewage Treatment and Disposal System (OSTDS) permit or to request a service related to your OSTDS.

Operating Permits

Operating Permit: Operating permits are required for aerobic treatment units (ATU), performance based treatments systems (PBTS), commercial septic systems, and industrial or manufacturing zoned (or equivalent) septic systems.

Median Annual Flood Line: Assessment to determine the elevation of the median annual flood line on a property to ensure compliance with OSTDS siting requirements.


Site Evaluation: Examination of soil, topography, and other site conditions to assess suitability for OSTDS installation.


Construction Inspection: Inspection of the OSTDS during construction to ensure it complies with the approved permit and design specifications.


Re-Inspection: Follow-up inspection required after the initial construction inspection. This may be an inspection after the system is covered, a re-inspection after a failed inspection, or an inspection to ensure proper installation and construction of a mound system.

Important Information About Local Ordinances

When applying for an Onsite Sewage Treatment and Disposal System (OSTDS) permit, applicants must comply with all applicable local ordinances in addition to state requirements. A permit issued by the state does not exempt applicants from adhering to local regulations. Applicants are strongly encouraged to verify that their proposed system design meets local requirements before submitting their state application. If a system design does not comply with local ordinances, the local municipality may deny their application, even if the state permit is approved. This would require the applicant to reapply with a revised system design that meets both state and local requirements, potentially causing significant delays in the permitting process.

New Construction Permit: Required for installing an OSTDS on a property where no system currently exists.


Existing System Permit: Issued for evaluating or permitting an existing system when there is a change in property use, ownership, or system function.


Repair Permit: Needed to fix a failing or damaged OSTDS to restore proper function.


Modification Permit: Required for altering an existing system, such as expanding capacity, relocating components, or changing the system type.


Abandonment Permit: Necessary to properly decommission and dispose of an OSTDS that is no longer in use.

A site evaluation assesses soil, topography, and other conditions to determine the suitability of the property for OSTDS installation. Applicants may choose to have the Department of Environmental Protection (DEP) conduct the evaluation or hire a private site evaluator.


Important:

If you choose to hire a private site evaluator, the evaluation must be completed before submitting this application. Additionally, the completed site evaluation must be uploaded as part of your application. Applications indicating the use of a private site evaluator without an attached site evaluation will be considered incomplete and cannot be processed.

  • Site Plan- Verify drawn to scale- show location of driveway, water lines, wells, surface water, other structures, etc


  • If property 5 acres or more- 1 acre to scale + overall plan showing location dimensions of the property and location of the 1 acre


  • Floor Plan with dimensions shown and usage of rooms for residential


  • Commercial systems require to scale floor plan with plumbing fixtures


  • Legal Description for property showing dimensions- Survey, Deed with legal, Subdivision lot/ block

  •  Sewer Non- availability Letter from Utility in Area


  • Agent Authorization- If agent signing paperwork- Building Contractor or Septic Contractor/ Plumber are exempt from requiring agent authorization


If DEP site evaluation required:

  • Enter Sunshine 811 Dig Ticket Entry- Site evaluation cannot be completed until cleared Sunshine 811.

  • Septic Tank Certification (Pump Out Certificate) from Septic Contractor- DEP4015 page 4
  • If tank cannot be used then mark To Be Abandoned across the form


  • Site Plan- must show shape of property and dimensions of property- Not required to be to scale- show location of driveway, water lines, wells, surface water, other structures, etc.


  • Must show location of existing system and proposed system


  • Legal Description for property showing dimensions- Survey, Deed with legal, Subdivision lot/ block

  • Sewer Non- availability Letter from Utility


  • 12-month water usage, if available (may not be available from some utilities)


  • Agent Authorization- If agent signing paperwork- Building Contractor or Septic Contractor/ Plumber are exempt from requiring agent authorization

  • Enter Sunshine 811 Dig Ticket Entry- Site evaluation cannot be completed until cleared Sunshine 811.

Modification Documents

  • Must show Existing Structure and Proposed Structure Building Information on Application –(Look in Ebridge/ Occulus for Previous permit info)

Basin Management Action Plans

Chapter 403.067, F.S. requires any new system within a BMAP to install an enhanced nutrient reducing OSTDS on lots one acre or less. Some BMAPs have additional requirements related to lot size. Please the link below to determine if your parcel is within a BMAP and requires an enhanced nutrient reducing system.
