Salem State Scholarship Thank You Note/Video Submission Form

Congratulations on receiving a Salem State scholarship! Please fill out all appropriate fields and upload your thank you notes and videos here. By accepting this scholarship, you authorize Salem State University to release pertinent information about you to scholarship donors/donors’ designee. If Salem State wishes to share your scholarship information publicly beyond just the donors, it will seek your advance permission before doing so. Future giving can be positively influenced when donors know of their scholarship’s impact on the scholarship recipient.
