This form is to request revisions to policies & procedures or to create a new policy.


PLEASE NOTE:

  1. Submit one request at a time.
  2. Requests will be approved according to COCA and NWCCU standards, and Noorda-COM Policy.
  3. All requests will be reviewed by the appropriate committee for approval or denial.
