Change of Residential Address Form
*This is only for USPS packages and letter mail. To forward UPS or FedEx packages, contact the shipping company.
Submitting a change of address form is necessary if you are:
- leaving campus permanently (e.g., graduating, moving off-campus)
- leaving for summer and expect to receive mail
- leaving campus for an extended period of time and wish to receive mail (medical leave or special circumstances)
DO NOT COMPLETE THIS FORM IF YOU ARE MOVING FROM ONE DORM TO ANOTHER ON CAMPUS.
