Change of Residential Address Form

*This is only for USPS packages and letter mail. To forward UPS or FedEx packages, contact the shipping company.


Submitting a change of address form is necessary if you are:

  • leaving campus permanently (e.g., graduating, moving off-campus)
  • leaving for summer and expect to receive mail
  • leaving campus for an extended period of time and wish to receive mail (medical leave or special circumstances)


DO NOT COMPLETE THIS FORM IF YOU ARE MOVING FROM ONE DORM TO ANOTHER ON CAMPUS.

The form must be completed and signed by the same individual who is changing the address. Mail is held for 10 days after the end of the semester. Mail received after that is returned to the sender unless/until a Change of Address Form is submitted.


By submitting, I understand that it is my responsibility to update address information with Mail & Package Services and to notify all correspondents when my address changes. I have also read the Forwarding Policies above.
