Vending Malfunction/Refund Request
Use this form to report a vending machine malfunction and/or refund request.
Vending refunds are applied directly to your 49er Account. Auxiliary Services will email you when your 49er Account has been credited.
There is a two-week time limit to claim a refund.
Refund restrictions:
$5 limit per visit
$10 maximum per person per academic year
Once submitted, your request will be reviewed for approval.
