CPCA Medi-Cal Health Navigator Project: Application Part II

Project Overview: The CPCA Medi-Cal Health Navigators Project (Navigators Project) provides funding for a variety of outreach and navigation services to various vulnerable populations who are potentially eligible for Medi-Cal. Subcontractors of the Navigators Project utilize funds to conduct various outreach activities in order to enroll and/or retain individuals within their communities.


Application Part II: CPCA reviewed all applications submitted in part one of the application cycle and identified potential awardees according to the scoring criteria leveraged from DHCS. The Association provided preliminary funding notices and allocations to potential awardees so that they may build a narrative, work plan, and budget that aligns with the preliminary funding allocation. CPCA invites the potential awardees to complete the second part of the application.


The review team will utilize the information submitted in both parts one and two of the application to submit final funding recommendations to DHCS. DHCS will approve subcontracts with community health centers and consortia, including the scope and budgets, prior to executing the subcontracts.

Deadline: The deadline to complete part two of the application is 5:00 P.M. PST on 11/3/23.


Instructions: Part two of the application consists of three components: 1) narrative, 2) work plan, and 3) budget plan. Please take your time and answer each question completely to build a project proposal that aligns with the proposed funding allocation shared by CPCA. Important Notes:

  • Only organizations who completed part one of the application and were selected as potential awardees based on scoring outcomes were invited to submit part two of the application. Only part two application submissions from organizations identified as potential awardees will be considered for funding.
  • Proposed funding allocations provided by CPCA to potential awardees may require applicants to scale down their proposed activities in part one of the application to create alignment between the funding available and scope of work.
  • Both part one and two of the application must be completed by the applicant to be fully considered for funding and to initiate a subcontract.
  • Final applications (both Part I and Part II) will be shared with DHCS for funding approval. Overall responses will be summarized and used for reporting purposes.
  • Subcontracts will be established with final subcontractors after DHCS approval.

Proposed Funding Allocation

Narrative

The narrative is used to describe what your organization plans to implement through the Navigators Project. Your organization should consider the target populations and any current navigation efforts when completing the narrative to express its interest and abilities to complete the project requirements.


Complete each question below to demonstrate the scope of activities that your organization plans to implement given the proposed funding allocation. This section is also intended to capture any changes that may result from scaling your project as needed between parts one and two of the applications.

As a Navigators Project subcontractor, your organization will be required to submit performance reports on monthly and quarterly basis. These reports will demonstrate the subcontractor’s capacity to conduct an objective assessment of project activities, collect, and report relevant data.

Work Plan

CPCA is requesting a completed Work Plan that provides a detailed scope of work that your organization and/or subcontractors will execute throughout the duration of the Project. A completed Work Plan includes full details for each of the three sections: 1) Planning & Start-Up, 2) Project Operations, and 3) Project Goals. It should list the plan, strategies, and activities your organization will use. The plan should list the activity, the target population for that activity, and the responsible party that will implement the activity. Review the Navigator Project application guide for additional details on allowable activities.


Please complete each section of the Work Plan and provide realistic goals, activities, and approaches based on your organization’s and/or subcontractors’ capacity and the proposed funding allocation. These details will be shared with DHCS for final approval before subcontracting commences.

As subcontractors build their work plan for part two of the application, they must consider and incorporate the following required activities.

  • Subcontractors must attend monthly and/or quarterly meetings that include DHCS, CPCA, subcontractors, and other stakeholders to share results, promising practices, and Medi-Cal updates for outreach and enrollment support to Target Populations.
  • Subcontractors must submit monthly data reports, quarterly progress reports, and quarterly invoices, including adequate documentation of all expenses. The reporting documents will be shared with DHCS and are intended to capture the key activities and outcomes of the subcontract and provide public transparency for this contract. Review the Navigator Project application guide for additional details on the data reports.
  • In addition, subcontractors must include virtual and/or in-person presentations to provide Medi-Cal outreach, education, and services to the Target Populations.


CPCA will use the Work Plan to refer to and monitor the work being completed. This document will need to be updated whenever the direction or guidance of the project changes. Any proposed revisions to the Work Plan by Subcontractors must be submitted to CPCA in writing.

Budget Plan

The budget plan must list the proposed funding allocation shared by CPCA to each potential awardee. Your organization should allocate funds for projected actual costs incurred for each fiscal year period based on your organizational needs to successfully implement the CPCA Medi-Cal Health Navigators Project. The instructions tab details what and how the budget plan should be completed. The descriptions tab is where each line item is detailed in a more specific manner. Important Notes:

  • Total funds requested in the Budget Plan must align with the proposed funding allocation provided by CPCA. A larger amount of funds cannot be requested via the Budget Plan.
  • Quarterly payments will be cost-based and issued upon invoice approval. Invoices will be submitted using a time and effort-based methodology.
  • Upon agreement execution, an up-front payment totaling 25% of the total award amount will be provided to help get the program running. Quarterly invoices will be subtracted from this balance. Once all of the prepaid funds are used, quarterly payments will continue.
  • Funds can only be used for the following allowable expenses: personnel staff, benefits, office expenses, minor equipment (less than $5,000), travel, training, outreach materials, media outreach, and approved community health center subcontractors (as applicable).
  • Capital purchases are not allowed.
  • Details submitted through the Budget Plan will be shared with DHCS for final approval before commencing the development of subcontracts.
  • Fringe cannot be more than 25% of budgeted salaries. The indirect rate cannot be more than 10% of total expenses for each organization.
  • All deliverables must be completed by June 30, 2025. The close out period for final reporting is July 1, 2025 to June 30, 2026. As a result, it is recommended that subcontractors align expenditures and yearly allocations according to number of resources required to complete the deliverables in a particular timeframe.
  • Expenditures identified in the invoice must be supported by all appropriate documents (such as receipts, proof of payment, time sheets, etc.) to substantiate that payment was made and is eligible for reimbursement of allowable expenditures.
  • Subcontractors attest that the funds will not be used for non-allowable expenses by providing two signatures on the budget plan. Please either add a wet signature by printing it, signing it, and uploading the scanned copy OR you may include a digital signature on the file that you upload below.
