Eddie Herr 2019 - Federation Coach Complimentary Accommodations Request Form

*IMPORTANT - PLEASE READ* If you will be the official Federation Coach for your country, and would like to request full complimentary accommodations (housing & meals) for the duration of the tournament, please fill out the form below. Full complimentary accommodations will include: - A hotel room at the Legacy Hotel at IMG Academy (host tournament hotel) - 3 Meals per day at the IMG Academy Campus Center Servery during designated meal times MEALS ONLY Requests: If you will be staying in off-site accommodations and would like to request meals only, please check the box "MEALS ONLY" below. (*IMPORTANT NOTE: The tournament will NOT cover the cost of your off-site accommodations if you choose to book at room else where during the tournament.) **Complimentary accommodations for the tournament will begin on, or anytime after, December 1, 2019, and will end the day after your last athlete has been eliminated from the tournament.** Should you have any questions regarding the request process or any inquiries regarding the accommodations provided, please send an email to eddieherrhousing@img.com.
