Toilet/Fixture Rebate Submission

Submit all required documentation to receive rebate. If toilets/fixtures are outside of the dates, or not clearly documented, rebates will not be issued.

 REQUIRED:

-Receipt for toilet/fixture

-PDF Rebate form filled-out / signed

-"Before" AND "After" pictures of toilet/fixture


("Before" picture must show old toilet/fixture BEFORE removal, "After" picture must show new toilet/fixture installed in the same location)

-I understand that a clear "before" AND "after" picture is required. If a "before" picture is not taken before toilet/fixture is removed, the rebate will be ineligible.

-Toilets must be installed prior to 1992. Fixtures must be installed prior to 2005. Any toilets/fixtures after this date will be ineligible.
