Mentoring Code of Conduct

Agreement Form


Faculty who wish to host rotation students will be required to:

  1. Agree to the guidelines in the Mentor Code of Conduct document
  2. Attend a workshop on program mentorship expectations before a student rotates and once every three years
  3. Maintain affiliate status in the program


Please complete this form to agree to the Mentoring Code of Conduct document.


Faculty must complete a workshop prior to a student doing a rotation in their lab. Visit the program website for upcoming workshop dates, and to register.

I agree to maintain Affiliate status in the Neurosciences Program*

Points are given for different roles in the program. Affiliate status reflects an accumulation of between 1-5 points, see table here (right-click to open the table in a new window).

I agree to participate in a mentor training session before hosting a student in my lab and at least once every three years.*
