Instructions:

Please submit this form to:

1) update information on your I-20; or

2) request a new I-20 if your I-20 is lost or damaged.

1) Updating information on your I-20

F-1 students must report any of the following changes to a school DSO (ISP staff) within 10 days of the change:

- Program of Study/Major

- Financial Information/Funding Source

- U.S. Address*

- Permanent Address (Address in your home country)*

- Phone Number*

- Name

- Dependent Information

* If you are updating your U.S. address, permanent address or phone number, be sure to update them in your WebSmart before submitting this form: https://phx-ban-ssb8.smccd.edu/

1) Go to My Profile.

2) Click "View/Update Personal Information." (You will get taken to a new page in a new tab).

3) Click "Personal Information."

4) Scroll down to the Address/Phone Number section.

5) Click "Add New".

6) Select "Mailing" for updating your U.S. address, "International" for updating your permanent address, and "Phone Number" for updating your phone number.

7) Enter your information by filling in the right fields.

8) Click "Add."

2) Request a new I-20

If your I-20 is lost or damaged, submit this form to request a new I-20. Please be sure to save a copy of your new I-20 and keep it in a safe place. ISP will review your request and send your updated I-20 by e-mail.

Counseling Appointment from Student Success Link

Here is how you can schedule an appointment with Carlos on Student Success Link:

Click on the “My Appointments” tab to schedule an Academic Counseling Appointment.
Click on the “Schedule an Appointment” button.
Select "Carlos Romero" as an individual counselor.
Select what topics you would like to discuss with your counselor.
Select the time you would like to meet with him.
Next, select where you would like to meet with your counselor. You may have options for phone appointments, or virtual appointments.

That’s it! You have scheduled your counseling appointment, and you will see a confirmation screen. You will also receive a confirmation email with your appointment details.

If you are unable to make the appointment on your own, you can contact the ISP (International Student Program) at 650-738-7021 or email skyinternational@smccd.edu providing the following information:

All the days and times that you can meet with him (Monday, Tuesday, and Thursday, between 9 am to 12 pm or between 2 pm to 4 pm. Wednesday between 12:30 and 7:30pm. )
Reason for the meeting: Be specific.
