This form is only for individuals in good standing with the company who have held a PAID position with Clean Vibes for 5 or more years or who have previously been an event manager for Clean Vibes. IF YOU PLAN TO WORK WITH CLEAN VIBES IN 2019 AND FIT THESE PARAMETERS, YOU MUST COMPLETE THIS FORM. This form is in lieu of an application and is meant to be an information update for 2019 as well as an opportunity to provide feedback on 2018. This form is not for long-time volunteers (no matter how many years you have volunteered with us) or crew that have only been paid employees for 4 years or less.

This form must be received no later than Monday, March 11, 2019. We will notify all applicants of our initial hiring decisions no later than Monday, April 15, 2019. Please note that some employees may receive notification of our hiring decisions for events in May prior to April 15th. Everyone will receive a response from us no later than April 15th.

Please note: Due to the online nature of this form, we recommend that you write your answers to the narrative section in a separate document and then cut and paste them into this form. We are very appreciative of the time and thought that goes into the answers on these forms. Writing out your answers and saving them in a separate document will ensure that your hard work will not be lost in the event that your computer loses its internet connection mid form or there is an error in the transmission of the form.
