Request for Placement Exam Accommodations

This form is to request disability-related accommodations on Placement Exams through Testing and Evaluation Services (TES) at UT and must be completed by the student requesting the accommodation(s). This information and the student’s signature are required so Disability and Access (D&A) staff have permission to speak with TES.

This form is only to be used to provide testing accommodations required because of a disability and supporting documentation from a healthcare provider must be submitted before this request will be processed. Non-disability related requests should be directed to TES. Requests are processed within 5-10 business days once all materials are received.

This form should NOT be used by students taking the UT Math Assessment (UTMA). The UTMA has been designed to be as universally accessible as possible. The test is untimed and can be completed using personal devices/technology, therefore reducing the need for most individualized accommodations. Calculation is a skill being tested as part of the UTMA, so calculators are not permitted on the exam.
