ACR Consumer Complaints

You have the right to submit your concerns about an ACR accredited facility's radiology imaging and treatment services. Most complaints are most effectively addressed directly with the facility performing the examination. Therefore, the ACR encourages you to address safety and quality of care concerns with the leaders of the accredited facility. Use the form below to contact the ACR directly to report a serious complaint if you feel that your concerns have not been adequately addressed by the facility. All fields below must be entered in order for the ACR to investigate the complaint. Please note that the ACR cannot follow-up on anonymous complaints. ACR will accept complaints through this form regarding CT, MR, Nuclear Medicine, PET, Ultrasound, Stereotactic Breast Biopsy, Breast Ultrasound, Mammography and Radiation Oncology.


Not all facilities are accredited with the ACR or for all modalities. The ACR cannot assist with complaints involving non-ACR accredited facilities or complaints involving X-ray, DEXA, billing and insurance issues, personal disputes, wait times or accuracy of radiologist interpretations/reporting and we strongly encourage you to follow-up directly with the facility on these types of complaints. In addition, criminal activity should be reported to the appropriate local, state or federal authorities.

The ACR is limited in what it can investigate by federal regulations. We are an FDA approved accrediting body, but we are not a legal authority that can impose consequences on individuals. The ACR can only investigate the facility’s compliance with the FDA Mammography Quality Standards Act (MQSA) Final Rules and ACR Mammography Accreditation requirements as they pertain to your complaint. MQSA law requires accrediting bodies to ensure that facilities meet all personnel, educational, and equipment requirements. It does not provide a method by which the ACR can evaluate the interpretation accuracy of a physician, billing practices, scheduling practices or compliance to state laws or regulations. Items such as these are beyond the legal authority of an accrediting body under federal law. Moreover, MQSA cannot assure that every examination will find all cancers early enough for successful treatment.


The purpose of ACR’s consumer complaint investigation process is to ascertain whether the concerns you identify arise out of any facility violation of MQSA requirements. The mammography facility involved will be notified of the investigation and will be afforded an opportunity to correct any substantiated deficiencies we find. Any failure or refusal to correct noted deficiencies will be reported to the FDA for possible further action, and/or possible revocation of their accreditation. The ACR will notify you in writing when the review is completed.
