On-Campus Animal Request Form

Consistent with the University of Puget Sound's Animals on Campus Policy, the university recognizes that there are circumstances in which animals may be appropriately brought on the university campus; however, animals must receive approval to be in university buildings.


Any approvals may be revoked if the Animals on Campus Policy is violated or if there are other changes in circumstances affecting the appropriateness of the animal being in university buildings, such as the operational needs of the department.

In order to seek approval, please first select the type of animal you are requesting to bring on campus from the options below, and complete all required fields prior to submitting the form. If you are requesting to bring an emotional support animal (ESA) on campus, you will receive a subsequent email with directions on how to provide medical documentation.

Important Information about Emotional Support Animals

An emotional support animal (ESA) may be a dog or cat (most common), small bird, rabbit, hamster, gerbil, fish, turtle, or other small, domesticated animal that is traditionally kept in the home for pleasure.


Generally, a dog must be at least 9 months of age to live on campus to assure that the dog is reliably housebroken, not disruptive to other residents, and has all of the shots necessary to make it safe to be around humans and other animals (that may be in residence).


Any student, who is asking for an emotional support animal accommodation, must have a psychological disability with symptoms that significantly impair their ability to function. The requested ESA must significantly reduce the disabling symptoms.


The efficacy of the ESA must be established before the request is made. Therefore, requests for ESAs should be made prior to the start of the semester. Students cannot bring a newly adopted cat or dog to campus.


  • An ESA makes it possible for people with severe and chronic disabilities to function.
  • Before you can request to bring an ESA on campus, you must have a documented psychological disability.
  • You and your mental health provider must demonstrate a history of using a specific animal to treat a psychological disorder.
  • Students who want an ESA need to explore the possibility of an ESA with their mental health professional when they are living off-campus. It will take some time for the treating professional to evaluate if the animal improves your daily functioning by reducing the symptoms of your disability
  • Students are not pre-approved for an ESA, and newly adopted animals are not considered an ESA, therefore you cannot bring a newly adopted cat or dog to campus for any reason.

Bringing pets or emotional support animals (ESAs) into university buildings is an exception rather than the general practice. If you are seeking approval to bring a pet or ESA into a university building, please explain the exceptional circumstances that exist so that the required approver has full information about your request.
