ATI Physical Therapy’s philanthropic efforts extend our commitment to quality care outside our clinic walls by giving back to our communities. If you'd like to request ATI's sponsorship for your next community event please complete the form below. Once the form is submitted, please allow up to 30 days for your request to be reviewed and processed. Please note: Sponsorship and event requests are reviewed on a first come, first serve basis and are not guaranteed. We can only accommodate the options listed on this form.
