This request form is to be used for all non-laboratory chemical pickup requests. Examples of non-laboratory chemicals include: paint, used oil, obsolete housekeeping chemicals, etc.

Once your request is received, EHRS will determine the schedule for removal.

Ex. Quarterly vendor removal or standard EHRS waste pick up.

A budget code may be required once the job is scoped.
