Medidata Partner Profile Creation or Revision Form

As a valued partner, we’d like to highlight your company in a "Partner profile" on the Medidata Partner page on our corporate website. Examples of Partner profiles can be found here: https://www.medidata.com/en/find-partner/ In order to create or update your profile on our site, please complete the following fields and attach your company logos as applicable. Medidata will automatically include any product accreditations attained on your profile page. **NOTE: IF YOU ARE CREATING YOUR PROFILE FOR THE FIRST TIME, PLEASE COMPLETE ALL FIELDS.** Updates to existing profile pages can be submitted through this form at any time, and only fields where updates are being made should be filled in.

Please list your company name exactly as you want it to be listed on your profile page. If you have a profile page and your company name has changed, please include the old company name as well in parentheses. e.g. ABC Research (formerly XYZ Technologies)

In approximately 200-400 words, explain what your company is, what it does, who it represents, anything you'd like your Sponsors to know about you, etc. This information will be visible to all Sponsors or Partners and others who are viewing the Partner Profile pages on the Medidata corporate website.

Upload a high quality logos as per these guidelines: LOGO COLORS: Please upload both (1) your company logo with primary company colors and (2) the white version of your logo LOGO FORMAT: The logos should be with TRANSPARENT BACKGROUND in both: (1) PNG, and (2) EPS/AI format LOGO SIZE: The ideal image sizes are 72 dpi and ~ 2560 px x 2560 px. Dimensions of rectangular logos should be relative to ~2560 px as the length of the image if possible. Minimum file size should be 1024 px x 1024 px. If you cannot upload a file to this form, please email the logos to your Partner Programs Manager or Director or your Partner Account Manager and complete all other relevant fields on this form.
