Litter Cleanup Wrap-Up Form

Please only enter information for your entire group if you are the group leader. Otherwise, report your own efforts only.
 
 
Please enter a park name if you cleaned in a park and a waterway name if you cleaned along a stream, river or lake. If you cleaned along a roadway or other location, please enter the address closest to that location.
 
 
 
 
Enter the same email that you used to create your My Volunteer Page account.
 
If you represent a group or organization, enter the name of the organization.
 

Please enter numbers only, except in the comments

If a field does not apply, enter "0".
 
Number of volunteers in your group under the age of 18
 
Number of volunteers in your group age 18 or older
 
If you are a group leader, enter all hours for your volunteers. For example, if you had a group of 5 volunteers pick up litter for 2 hours, you would enter "10".
 
Number of bags of trash that you or your group picked up
 
Number of bags of recyclable material you or your group picked up
 
Number of tires you or your group collected
 
 
Count both sides of the road you clean. For example, if you cleaned a half mile of roadway on both sides of the road, you would enter "1". If you only cleaned one side of the road, you would enter ".5".
 
Count both sides of a river or stream you clean, or count the perimeter of a lake. For example, if you cleaned a half mile of a stream on both sides, you would enter "1". If you only cleaned one side, you would enter ".5".
