NYSDOL Verification Form

NYSDOL will verify that the individual is a NYS Registered Apprentice and an Apprenticeship Agreement (AT 401) has been fully executed (signed by both sponsor and NYSDOL). NYSDOL will further verify that the college is on the NYSED Related Instruction Training Provider list.

Once verified the individual inputting the participant will be notified through email notification

Please fill out the form below to ensure participants are active state registered apprentices.

THIS FORM IS ONLY REQUIRED FOR STUDENTS RECEIVING FUNDING AS REGISTERED APPRENTICES. THIS DOES NOT APPLY TO PRE-APPRENTICE PARTICIPANTS AS THEY DO NOT REQUIRE NYSDOL VERIFICATION.

If the trade is NOT LISTED in the trade dropdown list, please contact apprenticeship@suny.edu.

If you do not receive a notification (usually one week) that a participant you inputted has been verified, please reach out to the lead campuses, Mohawk Valley Community College for Advanced Manufacturing and Other Sectors and SUNY Schenectady for Healthcare/Human Services.

Please confirm all details prior to entering information on this form as it will hold up verification.

Applicants will not be approved for funding should the participant be unverifiable through NYSDOL.

This form should be filled out by only SUNY Campuses.
