If you have previously submitted a document to Ho‘oulu Verification Services that could potentially facilitate your access to disaster relief assistance, we kindly request you complete this form. We are committed to exploring ways in which we can support you and your family.
BEFORE PROCEEDING WITH THIS REQUEST, please take note of the following information:
• Requested document(s) provided by Ho‘oulu are DUPLICATE COPIES and are NOT considered valid certified original documents.
• Requests for a copy of document(s) already submitted to Kamehameha Schools will remain pending until you are contacted by a Ho‘oulu Verification Services representative for supplementary information. This step is necessary to ascertain whether a copy of your document is eligible for release.

IMPORTANT NOTICE FROM MAUI DEPARTMENT OF HEALTH!
Mauiʻs Department of Health is currently offering a Temporary service to issue Certified Birth Certificates (and Marriage Certificates) - FREE of charge at the Maui office ONLY.
DOH REQUEST PROCESS:
• Begin by submitting your request online: https://vitrec.ehawaii.gov/vitalrecords/
• When prompted, opt for the Pick Up delivery method.
• Fill out the request form in its entirety
- Specify the reason for requesting a replacement. For instance, if it was lost due to wildfires (or a similar circumstance). This information assists DOH in evaluating the request.
- If your address has been affected by wildfires, please indicate that the address has been impacted by wildfires (or a similar event). This information assists DOH evaluating the request.
- If no identification is available, select "No." Alternatively, you can upload a photocopy of your ID.
- Opt for the "Pay Cash" option. Maui DOH will waive this fee when you pick up the document. The first copy is free, while additional copies will cost $4 each.
- Make sure to download the Order Receipt for your records. Maui DOH will contact you once the request has been processed, and the document is ready for pick up.
DOH PICK UP Procedure:
• Bring an ID or a photocopy of the ID.
• If you need a replacement ID, please contact the Department of Motor Vehicles and Licensing at: https://www1.honolulu.gov/duplicates/
• Bring your order receipt.
• For expedited service, contact Maui DOH with your name, order date, and order number. They will notify you when the document is ready for pickup.
DOH OPERATING HOURS:
• Maui DOH operates Monday through Friday from 7:45 am to 4:30 pm.
• Birth Certificate (BC) and Marriage Certificate (MC) pickups are available until 2:30 pm. Death Certificates (DC) must be issued by mail.
• The average processing time is typically 3-5 business days.
Please refrain from using our KS online form to request services from
The Department of Health or Department of Motor Vehicles.
