PMF Self Service Enrollment Request

Please completed this form to enroll in begin the enrollment process for one of the PMF Self-service platforms. Please provide as much detail as possible. Please read the SELF SERVICE USER SECTION of the "Proteomics and Metabolomics Facility - Policies and Procedures" document (https://biotech.unl.edu/PMF%20Policy%20and%20Procedures%20Doc_Final_20210713.pdf). This provides valuable information to help you know what to expect when using our facility. Within 24 business hours, you will be contacted with an approval email or follow up questions. (If you do not see an email, please check your spam folder or contact us at PMFhelp@unl.edu for an update on the status of your request.) Upon approval, you will be asked to complete and provide documentation of required EHS training and schedule a time to complete in-person instrument training.
