  • Complete and submit this form if you have applied for a undergraduate degree to be conferred this term and wish to continue undergraduate studies in a future term.

OR

  • Complete and submit this form if you have been awarded an undergraduate degree and wish to continue undergraduate studies in a future term.



STUDENTS IN CAREERS OTHER THAN UNDERGRADUATE: If you have applied for a degree in a career other than undergraduate and wish to pursue undergraduate studies in a future term, you must apply for undergraduate admission.
