Thank you for your continued interest in pursuing Yellowbelt certification through the Value Institute Learning Center at Dartmouth Hitchcock Health.

Please provide us with information on the 5S project, IDEA board, or Yellowbelt project you have lead, or the Greenbelt project or Blackbelt project you have been involved in.

If you are a member of a project team you may submit the same required files for certification as the project leader, but must answer the questions on the Yellowbelt certification form yourself. Do not copy and paste answers to the questions. We want to understand what you learned and how prepared you feel to undertake further improvement work.

Do not forget to attach the REQUIRED project files including; the project charter and another DMAIC tool you used during this project.

Once you have completed this form and attached the appropriate files, check the box for project submission. The VILC team will review the your submission for certification, and may contact you with further questions.
