OPTAVIA Bulk Client Transfer Request Form

Please note that this form is intended for OPTAVIA Coaches who wish to transfer multiple of their frontline Clients to a different OPTAVIA Coach.


Bulk Client Transfer Administrative Fee Schedule*

Please contact OPTAVIA Coach Support to pay any required transfer fees detailed below. Any required fee must be paid prior to processing the submitted request. 10-25 front-line entities (Clients, Leads, Inactives) = $25.00/ S$35.00/ HK$200.00 26-50 front-line entities (Clients, Leads, Inactives) = $50.00/ S$70.00/ HK$395.00 51-75 front-line entities (Clients, Leads, Inactives) = $75.00/ S$105.00/ HK$590.00 76-100 front-line entities (Clients, Leads, Inactives) = $100.00/ S$140.00/ HK$785.00 101-250 front-line entities (Clients, Leads, Inactives) = $250.00/ S$345.00/ HK$1,965.00 251+ front-line entities (Clients, Leads, Inactives) = $500.00/ S$690.00/ HK$3,925.00 *The administrative fees will be assessed based on the total number of Bulk Client Transfers requested within a commission week (Monday – Sunday). For example, if a Business Coach opts to give 5 front-line entities each to 10 downline Coaches, a $50/ SG$70.00/ HK$395.00 fee will be levied, since the total transfer of front-line entities equals 50.

Client Bulk Transfer Request List: PLEASE READ CAREFULLY

For Single Coach Recipient: Include the list of Clients you with to transfer AND their corresponding OPTAVIA ID OR For Multiple Coach Recipients: Include the list of Clients you wish to transfer with their corresponding OPTAVIA ID AND the Coach to which you wish to transfer each Client with each Coaches' OPTAVIA ID.

Please use one of the preferred file formats: .xls .xlsx .doc .docx, Google Sheets, or Google Docs.


Please DO NOT send image files such as .jpg, .jpeg, .pdf, etc. or Apple iWork files such as Numbers or Pages. We are unable to access these file types.

By completing this form, I understand that OPTAVIA will notify all parties of the changes. I also acknowledge that the Client reserves the right to select a Coach of their choosing. In addition, bulk transfers can take from one (1) to five (5) business days to complete entirely and will not be processed without the Coach paying the corresponding administrative fee. Bulk transfers will not be processed on the last day of the month. All volume that appeared prior to the date the Company receives the Bulk Client Transfer Request will not move with the Client, but remain with the previous Coach. Any orders placed after the transfer has been received and approved will be credited to the new Coach.
