FY24 HUB Reporting Form - University of Oregon

This form is for reporting fiscal expenditures on University of Oregon construction and consultation contracts between July 1, 2023 and June 30, 2024. Please fill out this form for each contract listed in the information request email. You can reuse the form URL if you have more than one contract to enter. You will be asked to provide the names of subcontractors, the contract value for each sub and the Historically Underutilized Business (HUB) status for each subcontractor. NOTE: This reporting is for the FISCAL YEAR expenditures. Please capture costs related to each subcontractor for only the time frame identified above. We do not want the full value of their contract reported, unless it was completely expended in this fiscal year.

HUB Certification Instructions (applies to all subsequent HUB sections)

The University of Oregon tracks certification for HUBs in four categories: Emerging Small Business (ESB) Minority Business Enterprise (MBE) Woman Business Enterprise (WBE) Service Disabled Veteran (SDV) For each HUB, select the certification(s) it has qualified for in the COBID program. Alternatively, if a HUB is not certified, but has self-reported an affiliation with one or more of the categories listed above, select the Self-Reported.
