COSF Program Submission Form
Any Oregon School District, Public Charter School, Education Service District (ESD), or Consortium that is eligible for the E-Rate Program, is eligible for the COSF Program.
ODE encourages any eligible entity that is pursuing or might pursue a special construction project through the E-Rate Program to apply to COSF for state-matching funding. The first step in the application process it to notify ODE of your intent to participate by completing this form.
Notices of Intent to Participate in the COSF Program for E-Rate Funding Year 2025 are due by December 13th, 2024
You may also use this form to request a consultation prior to starting the application process or to provide additional documents for a COSF application already in process.

A Connecting Oregon Schools Fund Award is contingent on the applicant receiving a Funding Commitment Decision Letter (FCDL) from USAC specifically for Special Construction. The award amount may be subject to prioritization if the funding requests exceed the allocation available to the program.
