Please provide a detailed description of the issue/damage. Be as thorough as possible trying to include info such as: Where on the furniture is the damage? How long has the issue been present? Did it present suddenly, or has it gradually gotten worse? What do you see, hear or feel that indicates an issue?

Submitting the proper photos of your issue is very important to processing your claim in a timely manner. Please ensure that all photos are clear and not blurry. IMPORTANT: Please click HERE for examples/instructions for each photo required.

  1. Picture of invoice
  2. Picture of I.D. tag (usually includes model/style number and serial number)
  3. Picture of entire piece of furniture
  4. Close up of damaged area(s). (At least one photo that is close enough to see what the issue is, but feel free to add multiple photos if needed.)
  5. OPTIONAL: Feel free to add a video of the issue if you feel like taking a picture doesn't properly illustrate the problem.
