Please fill this form out if you feel that you require a personalised evacuation plan in the event of an emergency, or if you have been advised to fill out a PEEP Request Form by staff at Leeds Conservatoire. For example, if you feel that you would require personalised assistance to leave the building during a fire alarm differing from the general evacuation information.

How your answers will be used:

Your answers recorded on this form will be sent to relevant staff with Health and Safety training in the SHE Services team, who will create a draft Personal Emergency Evacuation Plan (PEEP) for you. You will then be contacted to meet if necessary, read and sign the final copy, or suggest amendments if necessary. The PEEP will then be made available to relevant staff to use for emergency scenarios.
