This form is to be used by any SIBT future, current, or past student who wishes to lodge a Formal Appeal

• Formal Appeal must be lodged within 20 working days, from the date of the decision,
• Late applications will be considered at the discretion of SIBT,
• Students are strongly encouraged to contact SIBT prior lodging formal appeal and resolve the issue informally,
• All applications must include supportive documentation where possible.
