Please fill out this form to submit a reference check request(s) for a University Staff or Classified Position. NOTE: This form should NOT be used for Faculty or PRA positions, ONLY University Staff or Classified positions. If you have any questions on the reference check process, please contact HR.Recruiting@cuanschutz.edu.

Please do NOT use this form for Facutly or PRA positions. Only use this form for Classified or University Staff positions.

The finalized reference check report will be sent to this email.

Please provide the hiring manager's email if you would like the finalized reference check to also be sent to the hiring manager.

Please check with the candidate to ensure that they authorize the use of their phone number to conduct the reference check. The candidate can elect to not have the reference check sent to their phone. If they do so, please indicate N/A.

A candidate comparison report can be used with multiple candidates. This report compares multiple candidate ratings in one report. The candidate comparison report will come in addition with the individual reports.
