Meal Plan Waiver Form Information

For more information about the Meal Plan Waiver Policies and Procedures, please visit the Dean of Students website.


The meal plan waiver request form will be available during the below timeframes:

  • Fall Semester - July 8th - first Friday in September at 4:00 p.m. EST
  • Spring Semester - November 24th - third Friday in January at 4:00 p.m. EST


Failure to complete the form in its entirety or to provide the appropriate documentation could result in a delay or denial of your request. Students will receive an email notification to their louisville.edu account regarding the status of their request.
