Length of Service award missing from a members profile form

The purpose of this form is to notify the UK Headquarters Awards Team about a Length of Service award that you believe should have been awarded but has not been received yet and is not on a members profile.

Upon receipt of the request, the Scout Awards Team will aim to action this within 28 working days unless otherwise specified.
