Re-Certification for a Disability Parking Space and

Re-Application for a Block Limit Waiver

 

The City of Alexandria requires that everyone with a disability parking space re-certify each year whether he or she still needs the space, and if so, continues to meet the eligibility criteria of the program. The City also requires that everyone who received a Block Face Limit Waiver re-apply each year for the waiver.


Please fill out this form out in its entirety and upload the required photos or copies of your disability parking license plate or placard and vehicle registration by November 1.

Re-Certification of Eligibility Criteria

Please check Section 1 or Section 2, and Section 3, if applicable.

 

I no longer need a disability parking space.

 

I continue to need a disability parking space, and I have included copies or photos of the following with this form:


• A valid, nonexpired Virginia Department of Motor Vehicles (DMV) disability parking license plate or placard.


• A valid, nonexpired Vehicle Registration matching the name and street address on this form.

 

I will not be using this disability parking space for a period of three or more consecutive months within the year following the date of re-certification. (In such cases as this, the City will remove the disability parking signs for the period of time while you are gone, and the City will re-install the signs when you return.)
