Vendor Immunization Exemption Request
Ascension Associate Health and The Resource Group will evaluate your request. For both medical and religious exemptions, you must provide documentation.
- For medical and religious exemptions, your employer of record's Human Resource Department must provide you a letter on the company's letterhead stating that they are aware of the requested immunization exemption (by immunization name) and are supporting this exemption to be requested. This letter must be attached to the requested immunization exemption being submitted via this process.
- The employer of record is responsible for all medical/religious immunization exemption documentation should a regulatory agency request it when auditing any vendor access records to an Ascension facility.
- For approved influenza exemptions, In order to further mitigate the risk of transmission of influenza, all vendor representatives granted an exemption and who work in a clinical setting, will be required to wear a mask during the flu season from November 1, 2024 to March 31, 2025, in accordance with Ascension procedures and any applicable Federal, State, or Local laws.
If an exemption is granted, you may be subject to additional requirements; these will be provided to you with your approval notification.
