Youth Support Application FY25-26

**We have paused the Youth Support applications until December 1, 2025 for the next school year. Please return to this application at that date if you are interested in applying on behalf of your child.

Child Member must have Tailored Plan Medicaid.

Child member must be age 6-18.

Applications will be open in July 2025 and December 2025 only.

The Youth Support program offers up to $100 per year for Boys and Girls Club membership (if available in child member's county) or if not available, other child after school sports/activities or youth club memberships can be considered. Vendor approval is at the discretion of the program administrator.
