Beginning Monday March 3rd you will be able to exchange the old specimen collection cards for new cards by following these 4 steps:

1) Fill out the online Specimen Collection Card Exchange Form below.

2) Print a copy of the exchange form to send with your old specimen collection cards.

3) Mail the old specimen collection cards and the form to:

                         NBS Card Exchange

                         Oregon State Public Health Laboratory

                         7202 NE Evergreen Parkway STE 100

                         Hillsboro, OR 97124

4) Keep a 2-week supply of old cards on hand while we process your exchange. Once your shipment is received you can expect new cards to ship in 7-10 business days. Facilities can make more than one exchange if needed.

Considerations to be mindful of:

• DO NOT SEND COLLECTED SPECIMENS WITH THE KITS FOR EXCHANGE. Exchange kit shipments may not be immediately opened.

• Shipping methods with tracking are recommended. FedEx or UPS is preferred. For submitters without access to these services, you may ship via USPS.

• Refrain from using our PO BOX for card exchange shipments to limit transport delays to specimens and other critical mail.

• The pamphlets and envelopes have not changed and will not need to be replaced. You may request additional pamphlets and/or envelopes using this form.
