Prerequisite Waiver Form for AERE and EM Courses

This form is to request a waiver of stated prerequisites for a course. Prerequisite waivers should only be requested in extenuating circumstances. All students with unmet prerequisites (and who do not have an approved prerequisite waiver form) will be administratively dropped from the course or will receive an automatic "F" for the course regardless of the actual grade earned.

The student should complete and submit the form. The form will then be routed to your AERE Academic Advisor, the AERE Department Chair (or their designee), and the Course Instructor for evaluation. Final notification regarding the status of the request will be sent to the student (with their Academic Advisor copied).

Any questions regarding this form should be directed to the student's Academic Advisor.
