2021 Alumni Seal Awards Nomination Form

Since 1965, the Alumni Association has paid tribute to inspiring alumni who represent the values and spirit at the core of the Oxy experience. Seal Awards are given annually in the categories of service to the community, professional achievement, service to the College, young alumni achievement, emeriti faculty and alumna/us of the year. Please complete one form for each nominee, filling in as much of the information below as you are able. Nominees will receive top consideration when they are in their reunion year (for Reunion 2021, that means class years ending in 1 or 6). We especially encourage nominations of individuals who have done work related to fighting the COVID-19 pandemic. You may choose to nominate an individual for more than one award. The Alumni Board of Governors also reserves the right to consider a nominee for another award category.
