For permits with Public Tree Plans only, fields below can be left blank, unless there have been changes to an existing Plan. All other applicants must complete all fields, including selecting at least one of the following: Protection, Removal/Stump Grinding, Maintenance, or Planting.

Enter the total number of trees being impacted or planted. If a supporting document is being provided instead, you may enter, "See attached tree plan."

 

Add together measurements of all impacted trees. This number will be validated by the inspecting Forestry arborist.

 

Provide the Tree IDs using the Public Tree Inventory web map. If the trees do not exist in the inventory, or you are attaching a file for multiple trees, please enter "See attached file," or, "Not in Public Tree Inventory."

Provide specific information regarding the work you will be performing, including reason for removal or pruning. Example: "Remove 9" Red Maple, grind stump, restore ground to grade, for waterline project," or, "Plant Red Maple in city right of way in front of my home at 1111 E Broad St."

 
 

 

In making this maintenance/removal/planting application, I acknowledge that trees growing on city rights-of-way/park/public place fall under jurisdiction of the City of Columbus, Recreation and Parks Department, per City Code 912 and I agree to perform work according to the conditions listed at the bottom of this form:

 
 

  • All city, state and federal safety standards shall be followed.
  • All industry safety and professional standards shall be followed - refer to the Columbus Tree Technical Manual.
  • Permittee is responsible for proper traffic control including required permits.
  • Permittee is responsible for the disposal of any and all wood waste generated from the pruning and/or removal of trees. Site clean-up is required at the end of each workday.
  • Permittee is responsible for contacting all applicable utilities – OUPS at 811.
  • Permittee will provide routine maintenance (watering, mulching, basic pruning) to the tree at the time of planting.
  • Failure to comply with these conditions shall render this permit null and void and no future permits will be issued.
  • Permittee agrees to release, hold harmless and indemnify City from all injuries and damages that may incur relating to the work done pursuant this permit.
