2022-23 Isolation Housing Proctored Exam Submission

Please complete this survey if you would like your student to take an in-person, proctored exam while isolating on campus. Please answer all survey questions (#1-15) and submit your response at least 24 hours prior to the scheduled exam so that the proctor and exam are ready when needed.

Proctors will directly observe students throughout the exam and may contact you or your TA at the number you provide below if a student has a question during the exam.

We aim to offer exams in Isolation Housing at the same time as they are to take place for the rest of the class. If this is not possible, we will reach out to you to discuss whether an alternate time is acceptable.

Drop off physical exams at campus Isolation Housing, 1835 Hinman, east entrance.

Exam will be available on Canvas at exam start time
