Comprehensive Primary & Community Teams Workforce Development - Fund Application Request

This application form is to be completed by the applicant (employee) of eligible organisations to request a course/training under the Health New Zealand/Te Whatu Ora Comprehensive Primary & Community Teams (CPCT) workforce development fund.

If approved the course fees and some funding to cover expenses and backfilling of your role while you attend and or study will be paid to your employer. All payments for course, expenses etc must be claimed directly from your employer.

By making this application you agree and accept the conditions which include:

• I or my employer does not receive any other funding stream for me to participate in this training.
• I must complete all the modules, course work, and if a graded course, must receive a passing grade of the course being undertaken.
• I must complete the post course survey, in its entirety and upload the course certificate within 1 month of the course being completed.
