  • Name Change
  • Org Code Change
  • Update Appointment End Date


These changes cannot be processed locally and must be processed by H&S HR.

Please provide the new end date of this appointment. If this is a benefits eligible employee, please do not submit until:

  1. You have submitted and received an approved fixed term extension request from finance & HR (via this form)
  2. You have provided the employee a fixed term extension letter and uploaded a copy to your Box folder.

Please select if this is a temporary change (typically due to a vacancy) or a permanent change (change in structure, ongoing change in supervisor for students/temps, etc.)
