I have read the Student Records Policy as stated in the Southern Union State Community College Handbook.*

I request that my Southern Union and AUM transcripts, class schedules, and other information pertinent to my participation in the concurrent enrollment program be made available to both institutions.*

I understand that this request is only valid for this information to be released while I am enrolled in the concurrent enrollment program.*

I agree and commit to give written consent and authorize Auburn University at Montgomery, upon transfer as a full-time student, to release my academic record to Southern Union State Community College for purposes of credit evaluation.*
