This form is to be completed by a designated contact listed on the Student Quality of Service form. If you are not, this form will be disregarded.

The staff roster must be individuals who are working as paid staff and not volunteering (unpaid staff) with CMSD Community Partners and will be visiting and working on a regular basis at a school building.
