Once the examiners, date, and time for your oral examination have been established, this form must be submitted to your department for approval TWO WEEKS prior to the proposed date of your examination.

If you are in TGR status, skip the rest of the steps below and submit the form.

If you are NOT TGR status, continue on to Step 2.

Step 2: download, fill out and attach below the Application for Candidacy for Doctoral Degree (must be final version with advisors signature at the bottom under "Doctoral Dissertation Advisor")

Step 3: download, fill out and attach below the Doctoral Dissertation Reading Committee form (must have collected signatures of committee members)

Step 4: Attach proof of completion of CTL TA orientation. The screenshot or document MUST include your name.
