This form is for submitting new rotation proposals for GME to review. Please use this form only for proposing new reoccurring rotations. Please visit the Away Rotations page if your proposal is a non-reoccurring rotation.

Recurring rotations - Regular, repeating rotations to site outside of SHC and their affiliates, where House Staff rotate on an on-going basis.

Non-recurring rotation - Non-reoccurring, one-time rotations to site outside of SHC.

Contact your GME PM if you are don't know whether your proposal is a reoccurring or non-reoccurring rotation.

Before starting the new rotation intake form, please make sure you have:

• Goals & Objectives of the proposed rotation
• Program Letters of Agreement
• Site director name, phone #, email
• Host contact name, phone #, email

Note: Proposals for required rotations at new sites with which SHC has not yet established an affiliation will require review by the Graduate Medical Education Committee (GMEC).
