Developmental Pathways is currently offering a punch pass/voucher that includes 10 free visits to Sēb's Recreation Center (SRC) to any individual in services with DP. Please view the steps below to learn how to access this offer:

• Please complete the form below.
• A release form will be emailed to the contact emailed listed. This will need to be signed by the parent/legal guardian of the participant (or the individual, if they are their own guardian). *The voucher will not be approved until we have the signed release form.
• Please allow at least 2-3 business days for the request to be processed and emailed. You will then receive a confirmation email from Developmental Pathways stating whether or not your child has been awarded the vouchers.
• After you have received this email, the individual can then go into SRC and enjoy the fun!
• Please note, that this is a one-time offer available to certain individuals in service with DP. After you have used your first voucher, you are welcome to consider the possibility of requesting funds for more visits through your service coordinator or case manager depending on availability and the parameters of the services you receive.

Also, Developmental Pathways has the right to change or cancel this offer at any time. Please note, that the above guidelines have been put in place by DP staff and are not at the discretion of the employees at SRC.
